Looking For Transitional Care After Discharge?
How it Works
1. Get referred by your provider or sign up yourself to see a provider immediately after leaving the hospital. It's all covered by your insurance.
2. Once you sign up we'll text and/or call you, introduce ourselves, and you can schedule your telehealth post-discharge appointment. If you need a PCP, we will become your temporary PCP until we identify a permanent one for you.
3. We stick with you, 24/7 accessible via phone call or text, until all your post-discharge follow-up visits are complete, including specialist visits, blood draws, images, and procedures. We can schedule or reschedule visits for you and help arrange transport so you make it on time to appointments. We can also help plug you in with mental health resources and more throughout this period.
